29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Coiler Line: On or about June 10, 2022, Maintenance Employees working on the pay-off machine were exposed to struck-by and caught-in hazards while performing maintenance without machine specific lockout/tagout procedures for machines with multiple energy sources. Abatement certification and documentation required.

29 CFR 1910.212(a)(1): Machines were not guarded in order to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Rewind Area: On about June 8, 2022, Employees were exposed to ingoing nip point hazards, due to a lack of guarding, while operating the Coiler line due to open direct exposure to the take up Traverse Rollers. b) Rewind Area: On or about June 10, 2022, Employees were exposed to ingoing nip point hazards, due to a lack of guarding, while operating the P72 line due to open direct exposure to the take up Traverse Rollers. c) Rewind Area: On or about June 10, 2022, Employees were exposed to ingoing nip point hazards, due to a lack of guarding, while operating the P60 Line due to open direct exposure to the take up Traverse Rollers whenever it was operating. Abatement certification and documentation required.
