The following requirements apply to the use of all ladders, including job-made ladders, except as otherwise indicated: Ladders shall be used only for the purpose for which they were designed. a) On or about June 03, 2022, employees were reaching onto a roof top of a new tower structure utilizing a Werner 12 ft. step-ladder, trying to reach a height of above 14 ft. from the lower surface. Such a step-ladder was not being utilized for the purpose for which it was designed.

The following requirements apply to the use of all ladders, including job-made ladders, except as otherwise indicated: The top or top step of a stepladder shall not be used as a step. a) On or about June 03, 2022, an employee was framing a tower roof of a new commercial structure, accessing this area by standing on the top step of a Werner 12 ft. A-frame stepladder, to reach and climb onto the tower roof from the main roof of the new commercial structure.
