Condition noted on or about the 6-month period of Dec. 7, 2021 - June 7, 2022.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.    a) Jobsite located at 4500 Salmon St, Philadelphia, PA: The employer failed to ensure that employees were utilizing a fall protection system while performing roofing work on a low-slope roof with unprotected sides and edges and exposed to fall hazards of 13 feet above ground level. 29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Jobsite located at 4500 Salmon St,�Philadelphia, PA: The employer did not ensure that stairways or ladders were provided for breaks in elevation of 19 inches or more in that employees were permitted to climb the framing of the interior walls of a residential structure to access upper levels, potentially exposing employees to fall hazards of 11 feet above the next lower level.   Condition noted on or about the 2-week period of May 24 - June 7, 2022.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step.   a) Jobsite located at 4500 Salmon St,�Philadelphia, PA: The employer failed to ensure that employees were complying with the use requirements for portable ladders in that employees were using the top step of a stepladder and interior framed walls to climb up and down from the 3rd floor to 2nd floor framed structure while exposed to fall hazards of 11 feet above the next lower level.
