29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:   a) Worksite, residential structure -  On or about 6/10/2022, employees were walking and working on and around accumulations of construction debris throughout the worksite.  b) Worksite, residential structure -  On or about 6/17/2022, employees were walking and working on and around accumulations of construction debris throughout the worksite.  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) Worksite, residential structure -  On or about 6/10/2022, employees were exposed to being struck by lumber while working directly below over head framing.   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) Worksite, residential structure -  On or about 6/10/2022, employee was exposed to falling approximately 25 feet while installing roof rafters on a residential home.    b) Worksite, residential structure -  On or about 6/17/2022, employee was exposed to falling approximately 25 feet while installing asphalt shingles on a residential home.      Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
