(a) Location: 30 Beacham Street, Everett, MA: On or about 06/08/2022, construction debris and trash were accumulated throughout the work areas, exposing employees to tripping/falling hazards as well as delayed egress.

(a) Location: 30 Beacham Street, Everett, MA: On or about 06/08/2022, workers were exposed to a health hazard while using unsanitary portable restroom with full holding tanks, and toilet paper/debris scattered throughout. Due to these unsanitary conditions the portable toilets are not considered readily available for use.
