A lockout device was observed to have been utilized for other purposes than controlling energy. The lockout device was used to secure an employee locker in the breakroom.

An electrical junction box that was listed and labeled for permanent mounting only, was observed laying on the floor, powering a beer keg cooler and a Traulsen refrigerator in the employee breakroom. 2. An electrical four outlet box that was listed and labeled for permanent mounting only, was observed laying on a counter, powering a group of rechargeable power tool batteries.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: At the workplace located at 7312 Northwest Polo Drive, Kansas City, Missouri 64153, employees were exposed to electrical Shock, burns and fire hazards. The flexible cord and plug of the Folgers Select Brew Coffee System was observed to have inadequate strain relief. The cord had been pulled away from the plug, which exposed the underlying electrical conductors. The plug was plugged into the electrical outlet.
