29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch wide:  a. On or about June 08, 2022, employees were installing windows on a two-story single-family home from a 2-tier supported scaffold, during which time the working level of the scaffold was not fully planked or decked such that the space between adjacent units, and the space between the platform and the uprights was more than 1 inch wide; therefore exposing the employees to a 14'-9" fall hazard.

On or about June 08, 2022, employees were installing windows on a two-story single-family home from a 2-tier supported scaffold, during which time the scaffold and scaffold components had not been erected, moved, dismantled and altered by trained and experienced employees, nor while under the supervision and direction of a scaffolding competent person nor while under the supervision and direction of a scaffolding qualified person, and wherein the scaffold system was found to contain several visible predictable hazards such as having not been fully planked and having been missing the guardrails; therefore exposing the employees to a 14'-9" fall hazard.

On or about June 08, 2022, employees were installing windows on a two-story single-family home from a 2-tier supported scaffold, during which time the employees were not using a Personal Fall Arrest System nor was the three open sides of the working level of the scaffold protected with a guardrail system; therefore exposing the employees to a 14'-9" fall hazard.
