29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee wears appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about June 13, 2022 at jobsite 2503 Saddlebrook Drive, Naperville, IL 60564, the employer did not ensure that the employee working on ground level was using eye protection. Employee was exposed to eye injuries while trimmed, tree branches fell to ground and when using the wood chipper. Eye protection was not being utilized when engaged in these activities. No abatement certification or documentation is required for this item.

Employee was exposed to head injuries while employer was on the tree, trimming branches, and dropping them to the ground. No certification or documentation is required for this item.
