Providing such trainings/education would support the employer and employees in identifying and avoiding potential hazards such as struck-by injuries, lacerations, and amputations during job site operations.

On or about June 3, 2022, at the address of 3500 Weldon Dr. Cridersville OH 45806: The employer failed to ensure the employee furnished DeWalt model:DWE7490 serial number: 10-CT032547 10" table saw used to rip vinyl laminated flooring without a hood guard was inspected for safe operating conditions prior to employee use. Employees working in this condition were exposed to lacerations and amputation hazards.
