29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  (a) On or about June 9, 2022, at the jobsite located at 4820 S Whitnall Ave, Cudahy WI, an employee was removing material from the roof without any fall protection in place, exposing employees to an approximate 14 9/10 feet fall hazard.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   (a) On or about June 3, 2022, at the jobsite located at 4820 S Whitnall Ave, Milwaukee WI,  employees who were exposed to fall hazards were not trained on fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about June 9, 2022,  at the jobsite located at 4820 S Whitnall Ave, Cudahy WI, an employee accessing the roof to conduct roofing work from a ladder which did not extend at least three feet above the roof.
