29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. IMIS, LLC at 6087-6089 N. Liverpool St. Aurora, CO 80019: On June 13, 2022, the controlling employer did not ensure that subcontracted employees were protected from injury, in that four subcontracted employees working for Pacifico Builder LLC. were conducting framing operations at heights over 15 feet on a new residential construction without the use of any fall protection systems. This condition exposes to employees to head injuries and broken bones.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. IMIS, LLC., at 6087-6089 N. Liverpool St Aurora, CO 80019: On June 13, 2022, the controlling employer did not ensure that subcontracted employees were protected from injury, in that portable ladders, used to access the first level a residential home, had side rails that did not extend 3 feet above the upper landing, and was not secured. The employer did not protect employees against falls at heights over 15 feet. This condition exposes to employees to head injuries and broken bones.
