On or about June 14, 2022, with respect to a worksite located at 132 N Mall Drive, Appleton, WI 54913, the employer did not ensure that employees exposed to potential eye injuries while applying asphalt shingles with a pneumatic nail gun were protected by the use of protective eye wear.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about June 14, 2022, with respect to a worksite located at 132 N Mall Drive, Appleton, WI 54913, employees were exposed to fall hazards of more than twenty-five feet when the use of fall protection was not enforced while conducting roofing operations on a steep pitch roof.
