29 CFR  1910.305(b)(2):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  a) Meadow Gold Dairy, Gallon Bottler:  On or about June 14, 2022 and at times prior, employees were exposed to electrical shock hazards while working in and around exposed live electrical wires located near the controls of the of the gallon bottler machine.    b) Meadow Gold Dairy, Case Sealer:  On or about June 14, 2022 and at times prior, employees were exposed to electrical shock hazards while working in and around exposed live electrical components located near the operators position of the 3M-Matic Case Sealing System.

Section (5)(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  a) Meadow Gold Dairy, Purchase For Resale Cooler, Green Steel Storage Racks: On or about June 14, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading pallets of dairy products from the storage racks that did not display a plaque designating load ratings.   b) Meadow Gold Dairy, Purchase For Resale Cooler, All Steel Storage Racks: On or about June 14, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading pallets of dairy products from the steel storage racks that contained damage. The employer failed to ensure the keyed shaft presented a smooth edge.  b) Meadow Gold Dairy, Palletizer:  On or about June 14, 2022 and at times prior, employees were exposed to caught in hazards while working around the unprotected 2 1/2 inch diameter keyed shaft located on the palletizer. 29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a)  Meadow Gold Dairy, Pallet Jack Charging Station:  On or about June 14, 2022 and at times prior, employees were exposed to electrical shock hazards in that the electrical panel for the pallet jack chargers was not easily accessible.  Sufficient access was not provided in that the electrical breaker panel for the forklift charging stations  which was located approximately 47 1/2 inches from the ground and 41 inches behind the charging station shelf.
