29 CFR 1910.36(d)(1):Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: On or about June 15, 2022, the employer did not ensure employees are able to open the exit route door from the inside at all times without keys, tools, or special knowledge. Employees working in the facility were exposed to fire hazards from the secondary locks being used on the emergency exit doors in the following instances: A) The exit door located on North wall in the "factory" section of the facility has a panic bar that allows employees to open door from the inside, but also has a barrel bolt that locks the door from the inside. B) The exit door located on North wall that faces the west wall in the "warehouse" section of the facility has a passage doorknob that allows employees to open door from the inside, but also has a dead bolt that locks the door from the inside. C) The exit door located on South wall in the "Assembly Area" section of the facility has a panic bar that allows employees to open door from the inside, but also has a barrel bolt and a dead bolt that locks the door from the inside. This door is identified as the Emergency Exit main route. D) The exit door located on South wall in the "Pill Fill Room 1" section of the facility has a door latch that allows employees to open door from the inside, but also has a dead bolt that locks the door from the inside. This door is also framed in and blocked by an added residential door on the inside wall.

29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about June 15, 2022, the employer failed to ensure that operators of the Yale (Model #ERP030-04VT) and the Clark (Model #ESMI5) were competent to operate a powered industrial truck safely by successfully completing powered industrial truck training and evaluation.
