29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Church of the Living Word, 6099 Court Street Road, Syracuse, NY 13206, on or about 6/13/22: Workers were stripping and installing new roofing material to 30,000 square feet of roof, with a 4:12 pitch and a lower level to eave height of 10 feet and ground to eave heights of 12 feet, where personal fall arrest systems were rigged with slack in the lifeline that would allow a 6-foot free fall or contact with the lower level.
