On or about September 19, 2022, the OSHA 301 form equivalent the employer used for recording work-related injuries and illnesses for calendar year 2022 were not equivalent forms. The OSHA 301 equivalent form, for a work-related illness, case number 79654, illness onset date 5/9/2022 was missing the following: 1. Employee address, 2. Employee date of birth, 3. Employee date of hire, 4. Name of physician or other healthcare professional, 5. Where treatment was given away from the worksite, 6. Time employee began work on date of illness onset, 7. What the employee was doing just before the incident occurred. b. On or about September 19, 2022, the OSHA 301 form equivalent the employer used for recording work-related injuries and illnesses for calendar year 2022 were not equivalent forms. The OSHA 301 equivalent form, for a work-related injury, case number 96042, injury onset date 6/29/2022 was missing the following: 1. Employee address, 2. Employee date of birth, 3. Employee date of hire, 4. Name of physician or other healthcare professional, 5. Where treatment was given away from the worksite, 6. Time employee began work on date of illness onset, 7. Whether or not the employee was treated in an emergency room, 8. Whether or not the employee was hospitalized overnight as an inpatient, 9. What the employee was doing just before the incident occurred, 10. What object or substance directly harmed the employee.
