29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components were not removed from service: Location: 5 Topping Way, Chester, NJ a) Employees were exposed to fall hazards while conducting roofing operations approximately 20 feet in height without properly inspecting their fall protection systems that contained distinguished wear, damage and deterioration on or about 6/15/22. This further led to an incident which occurred on or about 6/14/22.

LM Home Improvements LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1925.501(b)(13), which was contained in OSHA inspection number 1518416, citation number 1, item number 1 and was affirmed as a final order on 06/04/2021, with respect to a workplace located at 25 Birchwood Rd., Denville, NJ 07834.
