29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, or had guardrails which blocked employee access to the cantilevered end.  Jobsite, side of building 2 -  On or about June 15, 2022, the approximately 24 foot long pump jack scaffold platform located approximately six feet above ground extended approximately five feet past its left support and right support and was not restrained to prevent tipping or equipped with guardrails to block employee access to the cantilevered ends.

Jobsite - On or about June 15, 2022, a Ridgid compound miter saw was being used by an employee that did not have the lower blade guard installed, exposing the employee to the blade and possible cut and amputation injuries.  b.) Jobsite - On or about June 15, 2022, a portable table saw onsite did not have the blade guard installed, exposing employees that use it to the blade and possible cut and amputation injuries.  c.) Jobsite - On or about June 15, 2022, a Hitachi compound miter saw had its lower blade guard modified where it was tied in the up position with a piece of wire and could not automatically return and cover the blade as designed.  LG Services, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1396884, citation number 1, item number 1  and was affirmed as a final order on August 16, 2019, with respect to a workplace located at 149 Portsmouth Ave in Strafford, NH.
