29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) Johnner Ward at 4200 North Interstate 90 Service Road Exit 61, Rapid City, SD 57701:  On or about and at times prior to May 11, 2022, the exposing employer did not ensure that employees were properly trained on the use of fall protection systems prior to being exposed to fall hazards while working on a low-slope commercial roof.  This condition exposed employees to fall hazards greater than six feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a)Johnner Ward at 4200 North Interstate 90 Service Road Exit 61, Rapid City, SD 57703:  On or about and at times prior to May 11, 2022, the exposing employer did not ensure that employees were protected from falls while conducting roofing operations on a low-slope commercial property.  This condition exposed employees to fall hazards greater than six feet.  Johnner Ward was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1356137, citation number 1, item number 1 and was affirmed as a final order on April 12, 2019, with respect to a workplace located at 2024 Main Avenue Southwest, Cullman, AL 35055.
