29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 18 West Bridge Street, Oswego, NY 13126, on or about 6/14/22: An employee of Pinnicle Builders was rip-cutting a section of wood with a circular saw that kicked back and severed the employee's finger. The employer did not report this incident to OSHA for 30 hours following the amputation.
