On or about June 16, 2022, the employer did not ensure that employees who were authorized to operate powered industrial vehicles were certified.  The employees had not been trained and evaluated as required. No forklift operators have been sent for training. The employer acknowledged that they did not perform any forklift training or certification for employees who operate the lift truck.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were exposed to crushing and amputation injuries from unexpended startup on the silica bagging machine.    No abatement is required for this citation item.
