29 CFR 1910.151(b): In the absence of an infirmary, clinic or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons were not adequately trained to render first aid: On or about 6/14/2022, employee engaged in oil and gas rig servicing activities sustained serious injuries of fall hazards resulted in broken ribs and hole in throat that required medical attention. The employer could not document that at least one person at this rig was adequately trained to render first aid with valid certificates in Basic First Aid and CPR.

Employees using hazardous chemicals, including but not limited to crude oil, diesel, gasoline, grease, hydraulic oil, transmission fluid, and PS-319 Solvent, were exposed to dermal and respiratory hazards due to the employer's current written hazard communication program not being updated in accordance with the revised Globally Harmonized Hazard Communication standard.
