29 CFR  1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On or about June 16, 2022, and at times prior to, an employee was exposed to struck-by hazards, while using a Ryobi drill press for metal, without using safety glasses for protection from flying particles.

29 CFR  1910.157(e)(3):The employer did not assure that portable fire extinguishers are subjected to an annual maintenance check, record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less:  On or about June 16, 2022, and at times prior thereto, an employee was exposed to a fire hazard, while welding and working in the shop, with fire extinguisher that had not been inspected annually.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:  On or about June 16, 2022, and at times prior to, an employee was exposed to a caught in hazard, while using a Ryobi drill press for metal, without having a guard for protection.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  On or about June 16, 2022, and at times prior thereto, an employee was exposed to a fire hazard while working in the shop where the oxygen and acetylene canisters were not stored separately.
