29 CFR  1910.22(a)(1):All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, in the warehouse boxes, pallets, trash, and debris obstructed passageways and walking-working surfaces, thereby exposing employees to slip and trip hazards.

29 CFR  1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, portable fire extinguishers on the office wall of the warehouse were not readily located by marking or signage and were not readily accessible in that they were blocked by a portable trash can, thereby exposing employees to fire hazards.   b) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, portable fire extinguishers in aisle 4 of the warehouse were not readily accessible in that they were blocked by  4-wheeled portable bin filled with cardboard boxes, thereby exposing employees to fire hazards.   c) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, portable fire extinguishers in front of receiving door 11 of the warehouse were not readily located by marking or signage and were not readily accessible in that they were blocked by wooden pallets and material, thereby exposing employees to fire hazards.   d) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, portable fire extinguishers in the south aisle of the warehouse were not readily located by marking or signage , thereby exposing employees to fire hazards.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) Commercial Works Inc., at the jobsite located 1299 Boltonfield St. Upper Arlington, Ohio 43221: On and/or preceding June 16, 2022, the employer failed to ensure a faceplate covered exposed wiring inside of a junction box in the warehouse, thereby exposing employees to electrical shock hazards.
