29 CFR  1926.152(a)(1): Containers other than approved metal safety cans or DOT approved containers were used for the handling and use of flammable liquids in quantities of 5 gallons or less:  Jobsite - On or about June 15, 2022,  a standard plastic 5 gallon gasoline container was used in place of an approved safety can or DOT approved container to dispense gasoline onsite. Employees used the unapproved container to fuel one of the air compressors being used at the site.

Jobsite - On or about June 15, 2022, employees were refueling a gasoline powered air compressor with an unapproved 5 gallon portable container and did not have a fire extinguisher onsite in case of a fire.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Jobsite - On or about June 15, 2022, employees performing roofing operations were not protected from a fall of over 17 feet from the eave and approximately 30 feet from the peak where no type of fall protection was being utilized and all employees on the roof were actively working.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Jobsite - On or about June 15, 2022, the aluminum ladder being used by employees for access to the roof was not extended at least 3 feet above the landing surface, exposing the employees to a fall of greater than 16 feet to the ground below.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.         Jobsite - On or about June 15, 2022, the employer did not ensure employees were following the manufacturer's specifications and limitations while using a RGC material hoist by allowing employees to climb the hoist, in disregard of the manufacturer's instruction to not do so, exposing them to falls of approximately 30 feet. Additionally, the employer did not follow the manufacturers instruction on assembling the track sections, where there were bolts missing from the splice plates to secure the track, making the hoist susceptible to collapse under load.

29 CFR  1926.200(h)(1): Accident prevention tags were not used as a temporary means of warning employees of an existing hazard.  Jobsite - On or about June 15, 2022, the RGC material hoist that was set up for use had a broken crossmember on the track and there was no accident prevention tag on it to warn employees of the hazard.
