On or about June 16, 2022, employees were observed during the roofing operations of a residential roof located at 6303 Acres Drive, INDEPENDENCE, Ohio 44131; the employer did not ensure the employees were protected from a fall that was approximately 9.9 feet above the lower level by use of a guardrail systems, safety net system, or personal fall arrest system.
