Employees were exposed to possible amputation hazards due to finger exposure in the danger zone (point of operation) during operations.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: a) Apache Steel: On or about June 28, 2022, and at times prior, the employer did not protect employees who walked and worked in areas adjacent to welding operations and plasma table operations. Nearby employees were exposed to ultraviolet radiation hazards due to welding operations and a CNC plasma cutting table in the main work area. Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Apache Steel Corporation: On or about June 16, 2022, employees were exposed to struck by and tip over hazards due to a lack of forklift training. The employer did not provide any initial powered industrial truck training or follow up training to their employees operating the forklift in regards to Powered Industrial Truck operations.
