29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Job site, 839 William Flinn Highway, Glenshaw, PA: On or about June 16, 2022, during roofing work, the employer did not ensure that employees used appropriate fall protection systems/devices on the roof, where fall hazards of up to at least approximately 20 feet existed.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 839 William Flinn Highway, Glenshaw, PA: On or about June 16, 2022, during roofing work, the employer did not ensure that the means of getting on/off the porch roof was safe in that a ladder, which was tied off, did not extend 3 feet above the landing and a grasping device, such as a grab bar was not provided.   b) Job site, 839 William Flinn Highway, Glenshaw, PA: On or about June 16, 2022, during roofing work, the employer did not ensure that the means of getting from the porch roof to the main building roof was safe in that a ladder was not tied off, it was not extended at least 3 feet above the landing and a grasping device, such as a grab bar was not provided.

29 CFR  1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use:    a) Job site, 839 William Flinn Highway, Glenshaw, PA: On or about June 16, 2022, during roofing work, the employer did not ensure that the a ladder, used to get on/off the porch roof was appropriate for use in that warning labels, safety instruction stickers, ladder weight ratings and other similar information were either missing, faded or illegible.
