29 CFR  1910.30(b)(1):  The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment:  a) On or about June 13, 2022, at the above addressed jobsite, the employer did not ensure an employee performing HVAC operations requiring the use of a fiberglass extension ladder to access the roof of the building received training on the proper care, inspection, storage, and use of ladders. The employee used the ladder during inclement weather with high wind conditions, thereby exposing the employee to a fall hazard of approximately fifteen (15) feet to ground level.
