29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: A water heating system was installed incorrectly, resulting in toxic gas emissions into Building 6534 air conditioning system. Building 6534 Boiler room: Lochinvar WAN201PM Condensing Commercial Water Heater serial number 2214 129032941 exhaust vent was not installed in accordance with the manufacturer's installation and operation manual. Employees were exposed to toxic gas hazards.
