In that, the safety latch of the hook was opened, and the hook showed signs of wear and deformation. b) At the fabrication shop located at 1138-1 Pondella Road Cape Coral, Florida: On or about June 17, 2022, the employer did not ensure that the stone slab lifting clamp attachment used to transport stone slabs, was inspected prior to use, and was removed from service when damaged or defective, exposing employees to a crush-by hazard. In that, the rubber pads and shell show signs of wear and deformation.
