29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. on or about June 22, 2000, on the south side of the site employees were performing roofing tasks on a roof that is greater than six feet above the ground without any being anchored off.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 26, 2022, on the southeast side of the site, employees were utilizing a ladder to exit the roof that was not three feet above the landing.
