29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. At the site located at 510 Orlando Ave, Winter Park, FL, on or about 6/17/2022, employees were exposed to fall hazards when working on scaffold. Employees would have to climb over the guardrail to access the ladder in order to egress the scaffold from the third level which was about 22 feet high.

At the site located at 510 Orlando Ave, Winter Park, FL, on or about 6/22/2022, employees were exposed to struck by hazards when working on above levels of the scaffold with tools and materials that can fall and strike employees working below, and toe boards were not installed on scaffold. Toeboard had more than 1/4" clearance, allowing tools and material to fall.
