29 CFR  1910.141(a)(3)(i): All places of employment were not kept clean to the extent that the nature of the work allowed:   Kitchen area: Employees conducted cooking, and cleaning operations on a daily basis. The employer did not provide a work area free of dirt, grease, and mycotoxins from mold species including Stachybotrys, Chaetomium, and Stemphylium. Employees were potentially exposed to various health symptoms including but not limited to upper respiratory tract illnesses,  internal lesions, dermatitis, and seizures.
