Employees were exposed to fall hazards while working at heights of approximately 10 feet above grade on a pump jack scaffold with deficiencies, including a lack of guardrails and not properly braced at the top and bottom.   b) 702 Kalmar Lane, Middletown, De 19709 (Side): On or about June 21, 2022, the employer failed to ensure that employees were trained to recognize the hazards associated with working on a pump jack scaffold and the procedures to control or minimize the hazards. Employees were exposed to fall hazards while working at heights of approximately 25 feet above grade on a pump jack scaffold with deficiencies, including a lack of guardrails, improper overlapping of platforms, missing base plates and mud sills, and not properly braced at the top and bottom.
