29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   a) Route 220 Southbound Lane and Quenshuckney Rd., Woodward Twp., PA: Employees attaching fall protection lanyards to concrete beam rebar protrusions at the beam level (walking working surface) as a means to transition from the ladder to the horizontal lifeline system were exposed to lower level ground surface contact in the event of a fall. The fall height at the south end bridge abutment was seven feet four inches and the fall protection stopping distance was approximately eight feet, on or about June 20, 2022.  Abatement Certification Required.
