29 CFR 1926.251(b)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and manufacturer: 2339 Hudson Terrace, Fort Lee, NJ: The single hook steel chain used for rigging to hoist a steel plate that weighs approximately 2,500 pounds did not have permanently affixed and legible identification markings. On 6/22/2022 an employee sustained an amputation to his left middle and ring fingers when the chain dislodged from the backhoe bucket.
