Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about June 15, 2022, employer did not ensure that frequent and regular inspection were conducted. Employees were exposed to struck-by hazards from a metal Framed Scaffold system that was leaned against a mobile hopper.

The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) On or about June 15, 2022, employer did not ensure that each employee working with tubular welded-frame scaffold had been trained on the recognition and avoidance of hazards while bundling together scaffold frames to move them to a different location.
