29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Worksite located at Bridge 1801, Gallatin, Missouri- Employee used a portable ladder to access the top of the bridge pier in which the ladder did not extend at least 3 feet above the upper landing surface, thus exposing the employee to a fall hazard.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Worksite located at Bridge 1801, Gallatin, Missouri a) Employee had climbed on top of the concrete bridge pier to disconnect the chain from the bridge and failed to use any means of fall protection. b) Employee had climbed on to of a work vehicle in order to perform work and was not protected from falling by the use of a guardrail system, safety net system, or a personal fall arrest system. The Omega Coatings & Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1493059, citation number 1, item number 2b, and was affirmed as a final order on 12-01-2020, with respect to a workplace located at Hwy 21 Bridge (Necedah, Wi) Over Wisconsin River Or 2299 Wi-21 19th Avenue, Arkdale, WI 54613
