29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Location - Pine Rd. Brentwood, NH facility. On or about June 23, 2022, modifications and additions were made to the Nissan Model MAP1F2A25LV forklift without permission from the manufacturer and data tags or decals were not changed accordingly. The forklift was equipped with 80.5 inch fork extensions that affected the capacity and safe operation while in use.

Brentwood, NH facility - On or about June 16, 2022, the operator of the Nissan forklift was dismounted and approximately 5 feet from the forklift and did not lower the load to the ground. The operator dismounted the forklift while it was suspending a car approximately 2 feet off the ground and was working under it.
