29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a)	2nd workstation from breakroom: The aisle leading from the computer workstation to the marked exit door and the aisle leading to the warehouse was blocked with cardboard boxes, trash containers and containers filled with product for shipment preventing emergency egress, on or about 6/22/22.  b)	3rd workstation from breakroom: The aisle leading from the computer workstation to the marked exit door was blocked with cardboard boxes and containers filled with product for shipment preventing emergency egress, on or about 6/22/22.  c)	1st workstation across from Outbound workstation: The aisle leading from the computer work station to the marked exit door on the south side of the building was blocked by cardboard boxes and containers filled with product for shipment preventing emergency egress, on or about 6/22/22.  d)	2nd  workstation across from Outbound workstation: The aisle leading from the computer work station to the marked exit door on the south side of the building was blocked by cardboard boxes and containers filled with product for shipment preventing emergency egress, on or about 6/22/22.  e)	3rd  workstation across from Outbound workstation: The aisle leading from the computer work station to the marked exit door on the south side of the building was blocked by cardboard boxes and containers filled with product for shipment preventing emergency egress, on or about 6/22/22.  f) Courtyard behind warehouse:  Emergency egress from the courtyard to the gate of the chain link fence was obstructed by overgrown weeds.  Weeds were observed growing over and around the gate to exit the area, on or about 6/22/22. 29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.   a) Receiving Desk: On or about 6/22/2022, a power strip that was powered by an extension cord, was plugged into another power strip that powered equipment, including, but not limited to, a computer, light, and cell phone charger.   b) 4th workstation across from Outbound workstation:  On or about 6/22/2022, A power strip was used to operate of heat gun.   c) 6th workstation across from Outbound workstation: Inspection Station #6: On or about 6/22/2022, a power strip was used to operate of heat gun.   e) 6th workstation across from Outbound workstation: On or about 6/22/22 a power strip powered by an extension cord was used to power equipment, including, but not limited to, a refrigerator and another power strip that powered a computer.   f) 7th workstation across from Outbound workstation: On or about 6/22/2022 a power strip powered by an extension cord was used to power equipment, including, but not limited to, a heat gun and phone charger.  g) 8th workstation across from Outbound workstation: On or about 6/22/22 a power strip was used to power a heat gun.   h) 9th workstation across from Outbound workstation: On our about 6/22/22 a power strip was used to power a heat gun.   i) Outbound workstation: On or about 6/22/2022, a flexible cord was daisy chained with three power strips to power equipment, such as, but not limited to, a computer, printer and label maker.   j) Outbound workstation: On or about 6/22/22 a four inch by four inch 120 volt electrical junction box was used as a pendant to power an extension cord that powered three power strips.   k) 1st -9th workstations across from Outbound workstation and 2nd and 3rd workstations from breakroom:  On or about 6/22/22 four inch by four inch 120 volt electrical junction boxes were used as pendants to power equipment, such as, but not limited to, power strips, heat guns, extension cords and small refrigerators.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
