29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): JOB SITE: 13333 Lakefront Drive, Earth City, MO 63045, on or about 6/22/2022, the employer failed to protect employees from having any part of their body in the danger zone during operating cycles of the Uhlmann B1240 Blister Packaging Machine. The employer permitted employees to operate the machine without guards, resulting in an amputation injury.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: JOB SITE: 13333 Lakefront Drive, Earth City, MO 63045, on or about 6/22/2022, the employer permitted employees to place their hands in the danger zone while removing jammed material on the Uhlmann B1240 Blister Packaging Machines. As a result of the process, an employee sustained an amputation injury. Employees were not provided with special hand tools for placing and removing material for the following equipment: 1) Multiple Uhlmann B1240 Blister Packaging Machines.
