29 CFR 1926.150(a)(3): Firefighting equipment, provided by the employer, was not conspicuously located: (a) On and before June 23, 2022, the employer did not ensure employees were protected from fire hazards, in that firefighting equipment was not conspicuously located while employees were engaged in construction activities, including cutting rebar with a STIHL T5 500i grinder. This condition exposed employees to burns and smoke inhalation.

29 CFR 1926.404(f)(7)(iv)(C)(1): Exposed noncurrent-carrying metal parts of cord- and plug-connected hand held motor-operated tools which may become energized were not grounded: (a) On and before June 23, 2022, the employer did not ensure employees were protected from electrical hazards, in that employees were directed to use a SKILSAW SPT77W, Serial Number: 805010431, that was not grounded adequately as the grounding pin was missing from the male connector of the equipment's power cord. This condition exposed employees to electric shock.
