OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: On or about June 22, 2022 at a job site located at 7432 Hills and Dales Rd NW, Massillon, Ohio 44646; employees were exposed to a fall hazard of approximately 14 feet while using the load area surface of an Equipter RB4000, as a means of access to install roofing material on a Mansard roof.

29 CFR 1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: On or about June 22, 2022, during the installation of roofing material at the job site located at 7432 Hills and Dales Rd NW, Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material. Creekside Metals LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and Face Protection), which was contained in OSHA inspection number 1505038, citation number 01, item number 01 and was affirmed as a final order on (01/28/2021), with respect to a workplace located at 2593 Oakway ST NW, N. Canton, Ohio 44720. Creekside Metals LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and Face Protection), which was contained in OSHA inspection number 1497670, citation number 01, item number 01 and was affirmed as a final order on (12/21/2020), with respect to a workplace located at 7154 Emerson, Massillon, Ohio 44646.
