29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)	On or about June 22, 2022 at jobsite located at 603 Castle St., Sandwich, IL 60548, an employee was not wearing a protective helmet where there was a possible danger of head injury from falling objects.  Employees were involved in the removal of asphalt shingles on a residential property and throwing them to the ground level.  This exposed the employee to struck-by hazards from the falling objects.    No abatement is required for this citation item.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a)	On or about June 22, 2022 at jobsite located at 603 Castle St., Sandwich, IL 60548, employees were exposed to eye injuries while conducting residential roofing activities with a pneumatic nail gun.  Eye protection was not being utilized while employees engaged in this activity.    No abatement is required for this citation item.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On or about June 22, 2022, employees were exposed to fall hazards more than 9 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing the activity.    No abatement is required for this citation item.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about June 22, 2022, the portable ladder used to access the upper framing roof platform did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to fall hazards.  No abatement is required for this citation item.
