29 CFR  1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about June 22, 2022, the area in front of a ladder used to access the roof was littered with roofing debris with protruding nails, exposing employees to tripping hazards and foot puncture injuries.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  On or about June 22, 2022, employees were exposed to fall hazards greater than 9 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing the activity.    No abatement is required for this citation item.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about June 22, 2022, the portable ladder used to access the upper framing roof platform did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards of approximately 9 feet.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
