29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Northern exit: Employees digging inside a trench approximately 6 feet deep were not protected from cave-ins. Employees were exposed to crushed-by and caught-in hazards. The Fletcher Construction company Ltd. was previously cited for violation of this occupational safety and health standard, which was contained in OSHA inspection number 1399979, citation number 1, item number 1 and was affirmed as a final order on August 5, 2019 with respect to a workplace located at a sewage project along the road in Atu'u village, Pago Pago, American Samoa.
