29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) 1284 Innox Road, Mechanicsburg, PA, New Residential Home - Employees were performing residential framing work while installing trusses on a residential home and utilizing a fall protection that was not rigged properly, resulting in a fall of approximately 8 feet 1 inches to the interior second floor, on or about June 17, 2022. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) GL General Construction Inc. - An employee suffered a right ankle fracture, on June 17, 2022 resulting in hospitalization. The employer failed to report this work-related hospitalization to OSHA within 24 hours. The employer reported this hospitalization to OSHA on June 21, 2022. No abatement certification or documentation required.
