29 CFR  1910.28(b)(13)(ii): The employer did not ensure that when work is performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, each employee is protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system. The employer may use a designated area when performing work that is both infrequent and temporary:  86 Keoke Beach Trail - The employee on the roof performing maintenance on the HVAC hood system was not using a means of fall protection and was exposed to a fall distance of approximately 13 feet to the ground below.

Establishment - An employee was not trained in fall hazards prior to accessing a roof exposing him to a fall height of approximately 13 feet 3 inches.

Establishment - The employer did not conduct an adequate workplace assessment to determine which Personal Protective Equipment (PPE) the employees should be using for tasks being performed. Specifically, to determine the need for fall protection on tasks that would expose the employee to heights greater than four feet in general industry and six feet in new construction.

29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.
