29 CFR 1910.178(l)(4)(ii)(A): The employer did not ensure that the operator operated the vehicle in a safe manner: (a) On or about June 27, 2022, and at times prior, an employee was exposed to crushing hazards while operating a forklift without wearing a seat belt. The employer did not ensure that employees operating forklifts such as, but not limited to, the Hangcha forklift in the shop area were protected from crushing, struck-by and ejection hazards by ensuring the use of a restraint system (seatbelts).

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) On or about June 27, 2022, and at times prior, the employer did not maintain a clear distance of 3 feet in front of the circuit breaker panel, located in the automotive shop area, in order to permit safe operation and maintenance of equipment.
